How to Request a Wish

To help make your wish come true, we need a few things to get started. We ask that you take the following steps:

Write a paragraph explaining your wish.
Complete the Request Form (see pdf)
Have your physician or medical care provider complete the Physician Statement (see PDF)
Include a photo of yourself (optional)

Once you have completed the four steps, send the packet to,

The Board of Directors will verify your eligibility, contact you with any questions, and get started on your wish.

Who is eligible for a wish?
Seniors 50+ in Contra Costa County with a chronic or life-threatening illness may qualify. Each wish is considered based on need, health, and available resources.
How do I refer a senior?
Anyone—family, friends, care teams, or the seniors themselves—can refer. Submit our online form or call, and we’ll walk you through the process.
What kinds of wishes are possible?
Wishes range from family visits and outings to simple joys like a favorite meal. Every wish is personal and tailored to the senior’s needs.
How can I help grant wishes?
Donate, sponsor, or volunteer—every gift brings hope and happiness to local seniors. Your support makes wishes come true.
